Adult Day Care

At Ultimate Care Adult Day Care, we provide a full-day program for adults 18+ in a safe, welcoming environment. Participants can engage in meaningful cognitive and social activities while benefiting from supportive resources that promote independence and enhance their quality of life.

Adult Day Care Participants must meet the following requirements:

• Have active Missouri Medicaid (MO HealthNet)*
• Be at least 18 years of age
• MO HealthNet Participants aged 18-63 must have a medical condition, diagnosis or disability that qualifies for nursing home level of care 

Note: MO HealthNet Participants aged 63 or older do not have to meet nursing home level of care in order to qualify for Adult Day Care.

*Not required for private pay participants

Adult Day Care is a Medicaid-funded program. The program is free for Missouri Medicaid (MO HealthNet) Participants.

We also offer affordable private pay options.

Yes! Participants are able to attend our day program AND receive home care services. Subject to authorization and program requirements.

1. Call our office at (314) 600-8879. We'll collect some basic information from you.

2. We’ll schedule an appointment for you to meet our director, tour our facility and conduct a pre-enrollment intake and functional assessment to determine whether our center is able to provide the level of service needed.

3. Here’s what to expect next:

For Medicaid to Pay:

If you already have a Home and Community-Based Services (HCBS) Care Plan:

• We'll submit a Home and Community-Based Services (HCBS) Care Plan Change Request for Adult Day Care to the Missouri Department of Health & Senior Services (DHSS).
• Expect a call from a DHSS Care Planning Specialist to verify and make changes to the request. If the request is approved, the specialist will initiate a new care plan for you.
• Please continue using your current care plan, back-up plan and support systems until your request has been completed. 

Please Note: Processing times for care plan changes may vary.

If you're new to the program and don't already have a Home and Community-Based Services (HCBS) Care Plan:

• We'll submit a new referral to the Missouri Department of Health & Senior Services (DHSS) for you to receive a Home and Community-Based Services (HCBS) Adult Day Care Waiver.
• A DHSS HCBS Assessor will call you to schedule a face-to-face assessment.
• At the assessment meeting between you and the assessor, prepare to answer a set of questions to determine if you are eligible for HCBS.
• If you are deemed eligible for services, the assessor will initiate a care plan for you.

4. We’ll schedule a start date, at which time you (or your responsible party) will complete necessary paperwork and review program guidelines. Once completed, you will be officially enrolled.
